What is EVLA?

Endovenous Laser Ablation (EVLA) is a varicose vein treatment method based on closing the vein from the inside using laser energy. The procedure is performed under ultrasound guidance by inserting a laser fiber into the vein with insufficiency. After the procedure, the treated vein is taken out of circulation, and blood flow is redirected to healthy veins.

How is EVLA Performed?

Endovenous Laser Ablation (EVLA) can be performed under general or spinal anesthesia in some centers. However, at our clinic, the procedure is carried out under local (tumescent) anesthesia, prioritizing patient safety and comfort. This approach reduces the risks associated with general anesthesia and allows patients to return to their daily activities quickly.

The procedure begins with the placement of the laser catheter into the target vein under color Doppler ultrasound guidance. The tumescent solution applied around the vein minimizes pain and prevents the laser energy from spreading to surrounding tissues. This ensures that the EVLA procedure is safe, effective, and comfortable.

Preparation Before the Procedure

Before endovenous ablation, you should obtain medium-compression (23–32 mmHg) varicose vein stockings that fit your leg measurements and extend to the groin. These stockings should be brought on the day of the procedure. After the procedure, the varicose vein stocking is worn immediately and should be used regularly for about 10 days. Trying the stockings at home a few days before the procedure helps ensure ease of use.

Since showers cannot be taken during the first 24–48 hours after the procedure, it is recommended to take a bath beforehand. A light breakfast is allowed on the day of the procedure, and regularly used medications, including blood thinners, can be continued. Comfortable and loose clothing is recommended.

Patients can walk on the same day after the procedure. Short-term use of compression stockings is recommended. Most patients can return to normal daily life on the same day. Follow-up examinations are performed with Doppler ultrasound.

EVLA may not be suitable for every patient with varicose veins. Eligibility for treatment is determined after a physician’s examination and Doppler ultrasound evaluation.
