The circulatory system in the human body is similar to the most perfect and complex highway network in the world. This highway has two main lanes: the arteries that carry clean and oxygenated blood pumped from our heart to our tissues, and the veins that carry the deoxygenated blood back to the heart by defying gravity. The pain, swelling, cramps, or color changes on the skin that we feel in our legs are the clearest harbingers of a traffic jam or structural defect in one or both of these two main lanes.
The golden standard in the treatment of circulatory system diseases is to “see” the source of the problem. When our patients present with leg complaints, they usually suspect varicose veins (venous insufficiency); however, sometimes the real problem can be an insidious blockage in the arteries feeding the leg. As a Cardiovascular Surgeon, my strictest principle in our clinic is to evaluate the leg as a whole and never plan any treatment without drawing a detailed “circulation map”. The miraculous technology that allows us to draw this map is Color Doppler Ultrasonography.
In this comprehensive guide, we examine the “Venous Doppler” and “Arterial Doppler” technologies, two completely different systems whose names are often confused with each other, what they do, and how they shape your personalized treatment plan with our surgical vision.
What is Venous Color Doppler Ultrasound and What Does It Do?
The word “venous” refers to the vein system. Venous Color Doppler Ultrasound is a zero radiation imaging technology that millimeter by millimeter displays the inside of the vessels carrying deoxygenated blood to the heart by using high frequency sound waves that the human ear cannot hear.
What Does It Do? Inside the veins in the legs, there are one way valves working like parachutes that prevent blood from escaping downwards. When these valves deteriorate, blood pools in the legs and causes Varicose Vein disease. With Venous Doppler:
- Detection of Valve Insufficiency (Reflux): We measure how many seconds the blood escapes backwards (reflux time).
- Measurement of Vessel Diameters: When deciding on laser or foam treatment, we calculate the diameter of the enlarged vessel millimetrically.
- Deep Vein Thrombosis (DVT): We precisely detect whether there is an insidious and dangerous clot carrying the risk of embolism deep within the leg.
Critical Surgical Note: In our clinic, Venous Doppler examination is absolutely performed while standing. Because varicose veins are a disease related to gravity; when the patient lies down, the intravascular pressure drops and the disease hides itself.
Why is Arterial Examination as Important as the Venous System?
Having varicose veins in the leg does not mean that your arteries are very healthy. Arterial diseases (Peripheral Artery Disease) usually occur with smoking, diabetes, high cholesterol, and genetic factors. Calcifications (plaques) forming on the artery walls block blood flow.
What Does Arterial Doppler Examination Do? If you get severe cramps in your calf while walking, if you have persistent coldness or non healing wounds on your feet, the problem is usually not varicose veins, but “lack of oxygen”. With Arterial Doppler ultrasound, we pinpoint these calcifications, narrowings (stenosis), or complete blockages in the vessels.
As a surgeon, before any intervention on the leg (for example, varicose vein surgery or wound care), we must make sure that the arteries are open and the tissue is fed with “clean blood”. Otherwise, the slightest aesthetic or surgical intervention on a leg without blood flow will lead to healing problems.
Frequently Asked Questions (FAQ) About Venous and Arterial Color Doppler Examination
We answer the most frequently asked questions by our patients regarding this comfortable diagnostic process where your circulation map is drawn, with our principle of medical transparency:
1. Is the Color Doppler Ultrasound procedure painful? Are needles used? Absolutely not. It is an examination performed entirely over the skin using a water based ultrasound gel. No needles are inserted into your body, and no medication or colored liquid (dye) is injected intravenously. It is completed with zero pain and maximum comfort.
2. Is there radiation in the procedure? Can pregnant women have it done? No, the device does not contain X rays (radiation) like an X ray or tomography. Imaging is provided entirely by harmless “sound waves”. For this reason, it can be applied with 100 percent safety even in pregnant women, babies, and kidney patients.
3. Can Venous and Arterial examinations be done in the same session? Yes, it can be done in case of medical necessity. In our patients presenting with mixed symptoms (both varicose veins and leg pain), the map of both highway systems (arteriovenous) can be drawn in detail in the same session in our clinic.
4. Do I need to fast or stop my medications before the procedure? It does not matter whether you are hungry or full for the examination of the leg vessels (lower extremities). You do not need to stop any medication you use regularly, including blood thinners. It is sufficient to wear shorts or loose pants that you can be comfortable in on the day of the procedure.
5. How long does the procedure take? A detailed examination of both arterial and venous systems of both legs, with positions taken standing and lying down; takes on average between 30 to 45 minutes depending on the anatomy of our patient and the complexity of the vascular structure.
6. I have a report taken at another hospital, isn’t this enough? Due to our surgical philosophy, it is essential for success that the Surgeon who will perform the intervention or treatment (laser, foam, etc.) sees the vascular anatomy with their own eyes. Standard radiology reports taken outside, usually lying down and written on an A4 paper, do not provide us with that 3 dimensional depth and risk analysis needed to plan a surgical procedure.
Venous Color Doppler Ultrasound Doctors and Arterial Examination Prices
One of the topics our patients who apply to the Op. Dr. Nebiye Tüfekçi Varer clinic with leg health complaints rightly research first is the diagnostic and treatment prices. In line with our medical ethical values and the legal regulations of the Ministry of Health, it is not possible for us to share a fixed price list on our website. However, we can transparently explain the criteria by which Doppler examination prices are shaped in our clinic:
- Scope and Detail of the Examination: Whether the ultrasound examination covers only the veins (Venous), only the arteries (Arterial), or both systems (Arteriovenous) is the main factor determining the pricing. Examining two systems at the same time naturally requires a more comprehensive effort.
- Single or Double Leg Condition: Since circulatory diseases are usually systemic, even if there is a complaint in one leg, it is recommended to check the other leg as well. Whether the examination is applied to a single leg (unilateral) or both legs (bilateral) affects the cost.
- Cardiovascular Surgeon Difference: The ultrasound performed in our clinic is not a standard radiological imaging performed by technicians. The fact that the procedure is performed personally by the Cardiovascular Surgeon who will plan your treatment and perform your treatment if necessary carries the quality of a “surgical mapping and risk analysis”. The fee you pay is not just the cost of using a device; it is the exact equivalent of the expertise and medical security that will directly manage the treatment of your disease.
The pain, heaviness, and fatigue you carry in your legs are not your destiny. Whether it is a venous insufficiency like varicose veins or an insidiously progressing arterial blockage; it is possible to learn the true condition of your circulatory system without feeling any pain. You are invited to the Op. Dr. Nebiye Tüfekçi Varer clinic to pinpoint the root cause of the problem with millimetric precision and plan your personalized “tailor made” treatment. You can immediately book your preliminary examination appointment to draw the autonomous map of your legs and step safely towards your health.
